It is important to emphasize that this representation is not comprehensive and does not claim to be. Rather, it reflects the inherent limitations of the database itself. Moreover, not all National Socialist medical crimes occurred within formal institutions; some were carried out at improvised or unofficial sites of persecution.

These include sites where crimes were physically carried out and victims were present, as well as administrative centers, laboratories, and – in the case of postmortem research – institutions where human remains were stored and examined, in some cases extending beyond the National Socialist period. Institutions associated with the broader system of National Socialist persecution (marked in blue):
These are sites where victims of medical crimes were hospitalized, detained, or killed. Although no medical crimes documented in the database are known to have been committed at these locations, their inclusion highlights the extent to which such crimes were embedded in the wider system of National Socialist persecution. This category includes, but is not limited to, concentration camps, prisoner-of-war camps, psychiatric hospitals, general hospitals, prisons, and the killing centers of the so-called "Aktion T4."
